Choose to Change is a specialist weight management service that helps adults and families make lifestyle changes that will enable them to lose weight and improve their health.

The Choose to Change programme is totally different to traditional weight loss programmes. It encourages lifelong changes to eating habits rather than short-term diets.   It helps people to overcome their barriers to weight loss, make sensible food choices and become more active.

CHOOSE TO CHANGE ADULTS. Specialist Adult Weight Management Service - What is it?

  • Choose to Change, delivered by ABL Health Ltd is a tier 3 Specialist Weight Management Service made up of a multidisciplinary team of Therapists, GPs, Weight Management Specialists, Nutritionists and Physical Activity Specialists.
  • The programme is for adults 16+ with a Body Mass Index of:
  • 40 and above
  • 35 and above with certain medical conditions or
  • 30 when pregnant
  • The programme is totoally different to traditional weight loss programmes. It encourages individuals to make small, realistic and effective changes that can be maintained in the long term, to enable them to lose weight and improve their health
  • It helps break down their barriers to weight loss, become more active and make positive food choices
  • The programme offers support for 12 months and encourages lifelong change rather than short-term quick fixes.

What can you expect from the programme:

  • Clients referred on to the programme will have an initial assessment. This is an opportunity to discuss personal barriers, eating habits, challenges and personal goals.
  • The majority of people will then start on a group but 1:1 support is available if a group is not suitable.? Home visits are also available if a client is house bound.
  • The service also offers specialist psychological therapy for clients with complex eating habits and issues around emitional eating.

  • A supportive and caring environment
  • Review of eating habits and updating of a monitoring diary
  • Session topics including portion sizes, food labelling
  • Tips on healthy eating
  • Guided weight loss advice
  • Information on how to maintain weight loss
  • Physical activity adapted for all abilities
  • Guidance on lifelong lifestyle and behavioural changes
  • Personal activity and nutrition goal setting at the end of each session

CHOOSE TO CHANGE FAMILIES

What is it?

  • Choose to Change, delivered by ABL Health Ltd, is a tier 3 Specialist Weight Management Service for children aged 4 and above with a Body Mass Index on or above the 98th percentile.
  • The programme offers 12 months of support to support the child to maintain/reduce their weight.
  • The programme offers families the opportunity to have one to one appointments every two weeks (in clinic or at home) followed by monthly follow-up appointments. An exercise group for all children in the service also runs alongside the appointments.
  • Staff work with families to offer a whole family approach and work with the family at their level of need.
  • In addition, extended support is offered to families and professionals involved in stages of (weight related) child protection and safeguarding.

What can you expect from the programme:

  • Families referred on to the programme will have an initial assessment. This is an opportuniy to discuss personal barriers, eating habits, challenges and personal goals.
  • The family will then be booked in for their future appointments at a time and day which is suitable for them.
  • The service also offers specialist psychological therapy for clients with complex eating habits and issues around emotional eating.

What can you expect from the sessions:

  • A supportive and caring environment
  • Topics including portion sizes, routine, behaviour change and food labelling
  • An individual approach based on the needs of each different family
  • Tips on healthy eating
  • Lifestyle Coaches have the freedom to adapt to sessions, and work with certain family members, such as parent only sessions to ensure every family element is supported

Working with children and young people to make small lifestyle changes encourages weight loss and health improvements, leading to a happier and healthier lifestyle for the whole family. Having the opportunity to work with the whole family also enables ABL to make an impact at a social level; hoping families pass health messages to family and friends.

ABL Health for Professionals

In addition, the service provides a training course for Health Professionals. The 3 hour Healthy Weight Champion Training Support individuals to confidently raise the issue of weight with both adults and children.

Learning outcomes of the course include:

 

- understanding the effects of overweight and obesity;

-how to identify and measure overweight and obesity in children and adults;

-barriers to weight loss and solutions;

 -how to sensitively raise the issue of weight and offer support;

- gain an in-depth understanding of the weight management services in the area and the referral process;
